The accident occurred on Jan. 20. A 53-year-old worker, who had been with the company for just over a year, was cleaning on the second floor, while a 47-year-old worker, who had been with the company for 10 years, was conducting maintenance when the roof collapsed. OSHA's investigation found that a truss supporting nine overloaded storage bins failed. The extra weight caused the bins to fall three floors into the center of the facility in roughly 30 seconds, according to OSHA.
The Omaha Fire Department rescued four workers that were trapped in the rubble. The injured workers and five others were taken to a local hospital with injuries.
Following its investigation, OSHA cited the company for one willful, one repeat and 11 additional safety violations. The company was placed on OSHA's Severe Violator Enforcement Program, fined $120,560.
